What you need to know about Female-to-Male (FTM) Chest Reconstruction
Female-to-Male Chest Reconstruction, or “Top Surgery,” is typically one of the first procedures that FTM patients undergo. This involves the removal of breast tissue to create a male chest reconstruction. There are two main surgical techniques that can be used to reconstruct the chest. Which technique is used depends on the amount of skin and breast tissue present.
For those with a lot of excess skin and breast tissue, the most common procedure is the bilateral simple mastectomy with nipple grafts, commonly known as the double incision (DI) surgery. This procedure does leave a scar on each side of the chest, but the scars are generally placed along the lower border of the pectoral muscle fold to help hide the scaring. While the scars are permanent, they should fade significantly with time. The area around the nipple (the areola) is reduced to an appropriate male size and is repositioned to an appropriate male location.
For those with minimal excess skin and breast tissue, another technique called a Subcutaneous Mastectomy is preferred as it removes breast tissue leaving only a small “key hole” incision. The clear advantage is that there is much less scarring.

OUR APPROACH TO FTM Chest Reconstruction
We can perform many of our FTM chest reconstructions as a Day Case, which means you’ll be able to go home in the afternoon and recover in the privacy and comfort of your own home. Of course if required, overnight stays are available as well.
Recovery typically involves wearing a compressive binder over the chest for approximately 1 week. We recommend no heavy lifting or strenuous exercise for approximately 6 weeks following surgery. Most of our patients return to relatively normal activity and sedentary work after 1 week.
All surgery carries risks which are important to understand so that you make a well-informed decision. Complications associated with FTM chest reconstruction include bleeding, infection, loss of nipple graft, alterations in nipple sensation, asymmetry, poor scarring, deep vein thrombosis (blood clots in the legs) and anesthetic complications.

A thorough discussion will occur during consultation with your surgeon including ways to mitigate risks prior to surgery and steps taken to treat complications should they occur.

What to Consider
Choosing to undergo any surgery is a big decision. Everyone affirms their gender in different ways, and that may or may not include surgery.
Surgery is permanent so you need to make sure it’s the right choice for you. Surgery doesn’t make you more or less trans.
What Criteria Do I Need To Meet Before Surgery?
- A history of gender incongruence (for 6 months or more)
- The ability to make a fully informed decision
- Ensure that any physical or mental health conditions are well managed
Letter of Support
You will need one letter of support from a mental health professional before having gender affirming top surgery. The letter needs to state that surgery is appropriate for you and is likely to help affirm your gender and reduce any gender dysphoria that may be present.
